You know these things, Jim S. I got the breach notice from a third party hired by ?Bristol Myers Squibb. I was briefly prescribed a medication. I did not request assistance to get the med, or join any research program. I did not sign any specific release of information in regards to this medication, medical condition, or treatment, with either the medical group or my pharmacy, CVS.?

I believe I used the medication either 8 or 18 years ago. Why does the pharmacy have any of my information in their system? I can’t get to anyone at BMS to find out why, or get it removed.?
