Don -- First, you must have a Premium group. Second, you must have defined a database table with a column of type "address." If you have both of these things, and open the table, the toggles between map and list views will be at the top of the page (immediately to the right of "Edit Table Properties").

Note also that -- for some unknown reason -- map view defaults to maximum zoom. I have to hit the "zoom out" button twice before my house comes into view.
