I am using the Bluetooth connection
to my Hearing Aids when it works, but otherwise the (not very
loud)-speaker
I have found "Headphone safety" in
SETTINGS>SOUNDS, but I can see what I have to do to tell it
to stop messing me about and leave the volume on maximum where I
set it.

Are you listening to the speakers, or is it sending a signal directly to your hearing aids?
The iPhone has a switchable sound-limiting setting that applies to ¡°headphones¡± ¨C which I suspect may include hearing aids as well as earbuds, airpods, and actual headphones.
Settings / Sounds and Haptics / Headphone Safety (near the bottom).
