Sorry Ralph away in Spain at the moment so no pictures. However it is just a box (which you could print) with the chinese aircraft connector -flying socket and male panel mount connector from AliExpress.

I just use small rectangular lidded plastic boxes which I drill for the motor wires then epoxy glue onto the motor. I fit a 4 pin chinese 16mm "aircraft plug" in the side and wire the flying socket to the cable. Has worked well for all of my motors and jobs for others.
