A nice feature of NVDA is configuration profiles (NVDA+Control+p). You can alter the behavior of NVDA so that it is configured differently for each application, so if you want it to speak the Caps only in Word, you can do this. I have set NVDA up in Outlook to not announce tables, which are traditionally only used for formatting html messages. It is a feature worth investigating so you can customize NVDA for each of the office applications and it will dynamically switch as you switch applications.
I did phone technical support at Freedomscientific. They told me that I had everything set correctly. It still does not work. So, what I did for now was to change the pitch from 50 to 70 to make it more distinct in pitch.

I do have NVDA and System Access on the computer that has a checkbox to say Cap letters. I will use these screen readers when I have a lot of documents to proof.
