This looks to me like the person (or email client) replying added a second instance of the #introduction hashtag to the message subject, that would create a separate topic thread as the subjects don't match anymore.

Do this, in that message with the double #introduction in the subject you posted in the screenshot, click on the More menu and select View Source.? Scroll down until you find the Subject: header.? I have a feeling it will either say,
Subject: Re: #introduction #introduction
or with no Re:,
Subject: #introduction #introduction
You would then need to contact that member and find out what they did which caused the double hashtag in their reply.

I second Bruce's suggestion of adding a group subject tag it makes things easier for email users who subscribe to more than one group.
