We have a closed group and, for the first time, I had a new member
apply for membership, and ... I found that the system had approved
him. ... There are two of us in that management loop, but I checked
with the other guy and he had not even been in the system.

Have you ever seen this happen before.

No.

I'd check the group's Activity log. It should show how the member was approved. Approval by email is supported (just Reply to the notification) so an accidental approval is possible even without visiting the group's pages.
