Does it make any difference if the Pending Subscription Message is the only Active message rather than the Welcome Message? Would the links for my approval/rejection then appear with that message? If I then approved the subscription would the welcome message then go out to the new subscriber?

Judith -- The Pending Subscription notice is not designed to be a user questionnaire. It is being used as a work-around for a feature that does not currently exist.
As such, you will have to manage this yourself -- pair up any response you get with the corresponding "needing approval" notification in order to approve these applicants. Note also that there is no guarantee that a pending subscriber will send you a response.
I realize that this probably fails to meet your expectations. A new user questionnaire feature has been suggested multiple times in beta, but for whatever reason has never been implemented.
