One invitation shows Status "Failed - 5.1.2".

What does this mean?

If you pasted the email address in the form:

Display Name <user@...>

look carefully for errant spaces inside the <angle brackets>. I've had that failing when creating a list to Invite using Excel and it can be difficult to see (depending on font).
