Or, you may have one or more thread with a hashtag carrying the Replies Unmoderated option.

?
And what can I do about it?

Review your hashtag list, opening the edit page for each until you find it. Then decide whether you want to remove that option from the hashtag, or remove that hashtag from current topics.
