Are you planning to manually invite them to the group? If so, you enter their name and email on the invite screen as needed.

The form would require for name as well as email address
Upon acceptance (if group is restricted) the system would enter the name as it does when entered in an invitation,
(and would ¡®tie to the group¡¯ in the same manner).
