The formula I follow is 1/4 cup low or no fat cottage cheese plus 1
Tbsp Flax seed oil per 25 pounds of body weight. Then I add 2 tbsps
or so of ground seeds. Your bowels will let you know if it's
toomuch. You can buy the oil wholesale from barcleans.com if you
tell them you are a cancer patient. Free shipping over $75.
You can use yogurt, but you need 3/4 cup per 25 lbs of body weight-
it gets to be quite a lot.
I recommend getting Dr. Budwig's cookbook. I found it worth every
cent.

As for recipe, I couldn't find any for whole flaxseed (how much)
for
sure so I started with 3-tablespoons to a cup of CC.
Is there a preferance between whole flaxseed vs the Oil that's
available? The oil would certainly be easier.
